Bowling-Medina Lanes

*PLEASE print and complete the attached waiver and bring to bowling on Monday, 10/5/2020 and Basketball on Tuesday, 10/20/2020 if you are participating in either one of these sports. The GUARDIAN must sign and date the waiver if the athlete is not their own guardian. (If you have already given a signed waiver to me either in person or by email – you don’t have to send another one. I DO NOT need one for each sport.)
